Symptom narrative

Patient here for TDaP booster and HPV #1. Pt endorsed anxiety in s/o needles. Did state remote hx of syncope w vaccines, though not in 'awhile'. Received fluzone at this practice 11/2023 without adverse event. Is unsure of last TDaP. Hasn't had HPV series. TDaP administered first, HPV after though both completed at 9:08 separated by ~30 seconds. Both in L Deltoid, spaced 1in+ apart. Approx 30 seconds after HPV vax patient endorsed 'feeling sweaty' and wondering whether she was 'about to faint'. RN at patient's side (seated in chair) when syncopal ~ 09:09/10 (estimate). LOC followed by ~15-20 seconds of irregular brief jerking movements. Total LOC time ~25-30 seconds. Once patient regained consciousness (~09:11), endorsed visual symptoms - at first endorsing blurred tunnel vision, then seeing 'spots' in vision. Patient provided cold packs and water, juice, and snack (pretzels) as patient was fasting at that time. This syncopal episode was unique in that she continued to experience cyclic lightheadedness and wondered if she was going to faint again. Per patient, the only other time that has happened was when she was eight in the setting of a vaccine though patient does not recall which - on that occasion she fainted more than once but was still able to go home. For the present-day, she felt cyclic presyncope symptoms after fainting but only did so the first time. In total, patient needed ~20-25 minutes seated with air flow (from fan), cold packs, and hydration/food before resolution of symptoms. Patient did state it took her longer to recover compared to prior instances and that usually she feels fine after a few minutes/interventions (i.e. cold pack, water). She cannot recall whether she was also fasting in those prior instances. It should be noted that today was the patient's first HPV vaccine, meaning she will need an additional two doses at some point in the future if deemed safe to do so.
