Symptom narrative

Medical assistant accidentally administered Proquad, and Kinrix instead of MMR and varicella individually. Assistant did not realize this until after patient left clinic. Mother of child was attempted to be reached 05/05/2026 x's 3, but unsuccessful. I was able to reach mother of child today and she states the patient is not having any side effects. Educated on any side effects to report. Clinic manager aware, and steps will be taken to provide education to prevent this from occurring in the future.
