Symptom narrative

After the shingles vaccine, about 30 seconds to 1 minute, the patient indicated she had slight pain radiating down from the injection site to her elbow and two fingers on right arm. This seems like the ulna nerve was affected upon vaccination. After 3 minutes the patient said she can feel the pain radiate up her neck to her jaw on the right side. After 10-15 minutes of observation the patient then reported pain radiating to her back right shoulder blade and reported feeling lightheaded and stomach upset. The patient is being monitored by the onsite nurse. We recommended having her lay down and elevate her feet to reduce the lightheadedness; however, the patient refused. Staff members indicated she has dementia, and it was clear she does because she didn't remember who vaccinated her 15 minutes after and if the vaccinator was a male or female. The patient indicated she has never had any serious reactions in the past against any vaccines, and this was her second shingles vaccine. First shingles vaccine, per State system, was January 6, 2026.

Allergies

Fluoxetine, gabapentin, nortriptyline, sulfa antibiotics, tizanidine
