Symptom narrative

The patient received Vaxelis and Prevnar vaccinations, administered in the right and left vastus lateralis, respectively. According to the mother, there was no redness or swelling at the injection sites in the 24 hours following the vaccinations. A firm swelling at the right thigh injection site was first noticed by the mother aprox 4 days after administration. However patient was not in mother's care for the previous 36 hours so mother is unsure when the swelling started. The swelling has not changed in size and does not appear to be bothersome to the patient. The swelling is assessed as a localized injection site nodule. The plan is to apply cool compresses to the area for 15 minutes on and 15 minutes off, as tolerated. A prescription for cetirizine 2.5 mg daily for two weeks
