Symptom narrative

Parent reports that child has history of fainting during vaccinations. Patient was positioned supine during vaccination and encouraged to lay there until asked to get up. A couple of minutes after vaccination mom reports "she is fine and can sit up". Patient sat up for a few minutes and then left the room while her sibling was being vaccinated. Patient sat outside of Exam room 3 in a chair with parent for about 15 minutes and then went to the waiting area with her mom and siblings. Patient was asked and she stated she felt good. At approximately 1415 registration requested that a nurse respond to the clinic B waiting area. Patient was found pale and diaphoretic. Nurses x 3 responded. Ice pack was applied to the back of her neck. Feet were elevated. BP is 108/70, HR 73. Patient responded after a couple of minutes and states she is feeling better. Patient was encouraged to sit in the waiting area for several more minutes. At approximately 1430 mom reports she is fine now and left the lobby. Patient reports that she is feeling better now.
