possible missed dose; This non-serious case was reported by a consumer via call center representative and described the occurrence of incomplete course of vaccination in a female patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Shingrix (received first dose of Shingrix in MAY-2021) and Covid-19 vaccine (received dose of Covid-19 vaccine on unknown date). The patient did not receive the 2nd dose of Shingrix. The patient experienced incomplete course of vaccination (Verbatim: possible missed dose). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 27-APR-2026 The patient had her first dose of Shingrix administered in 2021. Consumer stated she went back to the pharmacy for the second dose and was informed she had already received the second dose. Consumer stated she remembers having a COVID vaccine but not another Shingles vaccine. The pharmacy that she was going to now told the consumer she had not received her second dose of Shingrix. The consumer stated got a shingles shot in MAY 2021 at a now closed (withheld name) Pharmacy. Last year they told patient needed the second shot, but another (withheld name) pharmacist said already got it. Patient new pharmacy said not in data base for getting second shot. Unknown if the consumer received the dose or if the consumer received the dose later than the recommended dose schedule (possible improper dose schedule.)
