No additional AE/No PQC reported; Office Manager reported an improperly stored dose of ROTATEQ was administered to a patient 4/29/2026. No symptoms reported.; This spontaneous report was received from consumer or other non health professional and referred to a patient of unknown age and gender. The patient's pertinent medical history, drug reactions/allergies and concomitant medications were not reported. On 29-Apr-2026, the patient was administered an improperly stored dose of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) (dose: 2 mL, batch/lot number: 2208058, expiration date: 04-Jun-2027) for prophylaxis (Product storage issue). The vaccine was exposed to temperature excursion of 33.7 Fahrenheit degree for 40 minutes. There was no previous temperature excursion. No symptoms reported. No additional adverse event (AE)/No product complaint (PQC) reported.
