No symptoms reported; an expired dose of Gardasil 9 was inadvertently administered to a patient on 5/4/2026. Dose supported by post expiry memo.; This spontaneous report was received from an Other health professional (Medical Assistant) and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 04-May-2026, the patient was inadvertently vaccinated with an expired dose of Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) Suspension for injection (valid lot #2014790, expiration date was provided and confirmed as: 22-Apr-2026) 0.5 mL administered for prophylaxis by health care professional (expired product administered). Dose was supported by post expiry memo. No symptoms were reported (no adverse event).
