Symptom narrative

Redness, swelling that started two days after the vaccines. The patient said it is not painful, but the redness has spread over the last several days. I advised the patient to go to urgent care or his doctor to determine if it is an injection site reaction or an infection that would need an antibiotic. He did not report this to the pharmacy until 5-6-2026 at 9 am.
