Symptom narrative

Patient recieved HPV vaccine on 3/30/2026 in right deltoid. On 4/20/2026 patient reached out to provider complain of pain to right shoulder immediately after receiving the vaccine that continued. Patient by Dr. on 4/27/2026p/w right shoulder pain following HPV vaccine #1. Presentation and exam is consistent with SIRVA. Patient advised: - continue icing shoulder 2-3x a day - start naproxen 2x a day for minimum of 5 days - referral for PT - follow up with MD if no improvement in 2 months - next dose of HPV vaccine delayed 3 months. Start naproxen 500 mg tablet 1 tablet 2 times a day for 5 days
