Symptom narrative

Vaccination nurse reported that patient was experiencing numbness and tingling in his extremities later in the evening after receiving vaccines earlier that day. Medquadfi was administered in his right arm, Bexsero was administered in his left arm. Patient was admitted to the hospital for observation / rule out stroke. Additionally the patient had extensive bruising on his body from a recent fall (Saturday 5/2). Plans to follow up with patient and / or pt wife in 48 hours
