Symptom narrative

Immediately after vaccine administration, patient fell off exam table and hit back of her head. Patient was sent to the ER for evaluation. Per ER provider's note: Patient likely had a vasovagal syncope episode. Unlikely patient had total loss of consciousness or seizure.
