No symptoms reported; an improperly stored dose of ROTATEQ was administered to a patient 4/28/2026; This spontaneous report was received from an office manager and refers to a specific patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 28-APR-2026, the patient was vaccinated with improperly stored (33.7 F, 40 minutes, no previous temperature excursions) rotavirus vaccine, live, oral, pentavalent (ROTATEQ) oral solution at a total dose of 2 mL, administered by oral route, lot #2208058, expiration date: 04-JUN-2027, administered for prophylaxis (Product storage issue). No symptoms reported (No adverse event).
