Symptom narrative

Patient had increased soreness and stiffness in the arm that vaccine was given in. Patient reported that it was due to the administration site being too high in the arm. Patient did not need any further medical assistance as of 5/7/26. Due to this, I retrained all staff on proper vaccine administration.
