Symptom narrative

diluent lot number and expiration date, the caller mentioned that the print of the lot and expiration "looks pale and could be darker; No additional AE.; an expired dose of MMR-II that was administered to a patient.; This spontaneous report was received from a Nurse 07-May-2026 and refers to a 19 months female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 06-May-2026, the patient was vaccinated with an expired dose of Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II), injection (route, dose, and dose number were unspecified) (lot #Y008592, expiration date: 30-Apr-2026) for Prophylaxis. The vaccine was reconstituted with sterile diluent sterile diluent (BAXTER STERILE DILUENT) injection, prefilled syringe, (lot #Z007107, expiration date: 21-Feb-2028 (route, dose, and dose number were unspecified) for Prophylaxis. Confirmed no symptomatic adverse events. The nurse mentioned that the vaccine did not have any temperature excursions. While obtaining the diluent lot number and expiration date, the caller mentioned that the print of the lot and expiration "looks pale and could be darker"(Physical product label issue) (conflicting information also report as reported confirmed that this is not a PQC and all the company diluents look like this by the same reporter).
