Symptom narrative

2-21/2 weeks in 6/20225 Left great toe and 2nd toe top of foot on that side and lateral area foot become purplish/red with patchy itchy, flaky skin. Deep itch on bottom of foot. Continued after treat as fungal through 10/2025 then referral to Dermatologist-seen in Jan 2026 had spread to hands and some on bottom of right foot. Treated with steroidal ointments and creams- got worse developed pinpoint rash chest abdomen then to fingers, legs, arms and right ear. Just 5/7 treated with po Ivermectin and cream to repeat in 2 weeks.

Autoimmune symptoms but no definitive diagnosis/ ? Lymes 1993/mononucleosis x 2

PRN Meds: Albuterol inhaler/ Allegra Medication: 1) Symbicort Inhaler 2) Metformin 500 mg daily 3) Flonase (nasal spray daily)

Shellfish, Wheat, Morphine, Doxycycline, PCN,

4/30/26 Autoimmune panel 5/5/26 punch biopsies at Dermatology office Number of office visits and calls to PCP and Dermatology office.
