Symptom narrative

State immunization was printed by another staff member under patient's nickname. Writer was asked to give the vaccine for patient. State immunization for patient showing shingles vaccine was due to be given. Shingrix vaccine given to patient. After vaccine given, other staff member pointed out that there were actually two State immunization for the same patient. One was patient's legal name, and one was patient's nickname. Patient was in fact given a total of three Shingrix vaccines in her lifetime.
