Symptom narrative

Patient noted to have rash/reaction at the right upper arm following vaccination, characterized by erythema, swelling and significant tenderness at the injection site. Patient demonstrates wincing with mild palpitation/touch due to pain. Swelling radiates proximally toward the right shoulder. Area noted to be red and inflamed. Patient denies drainage. No fever noted at time of visit; however, patient reports taking tylenol every 6 hours for symptom management. Patient advised to go to Emergency room for evaluation and treatment. Patient agreed.
