Plan and disposition: Pt was offered a next day appointment for rash assessment. Pt declined. Pt advised to call if rash progresses. Pt advised to go to ED if has swelling and SOB. Chief Complaint: Patient, 54 y.o. female calling with complaints of localized rash. Pt is asymptomatic of pain or itching, swelling and SOB. Disease Management Program Membership: None Emergent Symptoms: Patient denies/is not having any difficulty breathing, speaking or swallowing. No swelling of tongue or mouth. Fever: fever- 100 degrees Onset of rash: Today hour(s) ago Signs and symptoms/ROS: flat, red, and blanching Location of rash: arm neck Last menstrual period: postmenopausal Pertinent Hx: PT is in the process of being treated for a bilateral rash across along both sides of neck. Space where thyroid was rash free but now is red. Allergies: Patient has no known allergies. Current home treatment: none. Effectiveness of current treatment: N/A. Pt states rash has faded around bilateral brachial pulse. Pt states rash occurred in front of neck after taking Levothyroxine. Also have rash on breast and back. Rash is not raised or itchy. Red patch around injection site and red patch in front looks like 3 circles across her arm. Brachial pulse - solid circle and dotted rash down to her wrist. This is almost gone. Pt received shingles on 5/5. On 5/6/2026 did not feel well. On 5/7/2026. Rash occurred while she was looking at her skin this morning. Pt is aware that if swelling and SOB present pt to go to ED or urgent care. Pt stated she will not go to ED for treatment as she is in the field and is aware of what to do if urgent symptoms present. Pt was offered a next day appt but declined. Pt advised to c/b if rash progresses. Called pt back 30 minutes later and pt reported "rash seems to be fading."
