Pt came in for additional vaccines, but presented with a rash on his back, back of arms and chest near shoulders. Was given MMRV, HPV and meningococcal previous day. Pt reported intermittent itching. May also have been exposed to cat hair, which he is allergic to. RN requested clinic provider assess pt. Provider stated it seems like a mild urticaria and advised pt to take OTC antihistamine. RN advised pt to continue to monitor for severe allergic reaction and go to ED if he develops sever symptoms. Parent also brought in (withheld) vaccine record aftr the fact indicating he had already had 2nd dose of MMRV. So he received an extra dose. Hep A (2nd dose) and Polio (4th dose) not given because (withheld) record shows complete.
