Symptom narrative

The immunization was given intramuscularly into the left arm in an arm muscle below the deltoid. The patient reported pain in the arm. The patient came back to the pharmacy about three hours later reporting pain. I followed up with the patient by phone about 6 hours after his shot to check on him and he stated he was resting. I informed him I would check on him again during my next shift in a couple of days and he said thank you.
