Large bruise Right Thigh reported by parents 4/15/26 at 10:22 difficulty ambulating. Appointment scheduled 4/17/26 with PCP to assess. Parents cancelled appointment morning of 4/17/26 reported improvement and took patient to local urgent care same day at 11:45am. Documented: bruise and redness of right thigh after vaccines 10 days ago. Initially was hard lump/bruise that has now flattened and spread out. Was sore for a few days. Now has a fine red, warm area over the bruise. There area is a little itchy but not painful. No fevers or ill symptoms. She is now back to full activity and no pain when pushing the area or with activity. Mouth: Moist mucous membranes. CV: Regular rate and rhythm. No murmur. Distal pulses 2+ and equal bilaterally. Capillary refill is 2 seconds. RESP: Nonlabored breathing. Symmetric with good aeration bilaterally. Clear to auscultation bilaterally - no wheezing crackles or rhonchi. MSK: No obvious swelling or deformity. No tenderness. Normal range of motion of all extremities. +5/5 motor strength. No clubbing, cyanosis, or edema. SKIN: Normal for ethnicity. Warm. Intact. Right thigh with central lateral area of bruising about 5x5 cm. There is a fine red rash over the area that is slightly warm and blanches. The area is not tender to firm palpation and there is no induration at this time.
