Symptom narrative

Patient reports large oval swollen area that was pink, hot and itchy in 24 hours. Patient iced the area and took Tylenol for achiness immediately. Patient reports the left arm got progressively worse and continues to be painful and has difficulty using it. Patient reports taking Motrin, doing physical therapy for 4 weeks, water aerobics for 2 weeks and taking muscle relaxers.

Other medications

Rosuvastatin, Escitalopram, Bupropion, Magnesium, Vit B-12, Multivitamin

Allergies

Mold, grasses, dust, ragweed, pine trees and dust.

Lab data

Patient reports x-ray was taken and MRI is scheduled.
