Symptom narrative

Nurse called and reported a 16 year old male received an extra dose of GARDASIL 9. The first dose was 1/15/2024, 1/28/2025, and third dose on 2/11/2026.; No side effects or symptoms.; This spontaneous report was received from nurse and referred to a currently 16-year-old male patient. The patient's pertinent medical history, past medication, drug reactions/allergies and concomitant medications were not reported. The patient was vaccinated with human papillomavirus 9-valent vaccine, recombinant (GARDASIL 9) injection 1DS x 10 syringes, 0.5mL/three doses, for prophylaxis (route of administration was not provided), the first dose on 15-Jan-2024 at age of 14 years old (batch/lot number and expiration date was unknown), the second dose on 28-Jan-2025 (batch/lot number and expiration date was unknown), and the third (extra) dose on 11-Feb-2026 (batch/lot number: Z008509, expiration date: 23-Jul-2027) (Inappropriate schedule of product administration). No side effects or symptoms (No adverse event).
