05/06/2026 Patient presented to the clinic for a physical. Per database, patient was due for her 5th Dtap. Provider ordered Dtap. Vaccine was administered to the (RVL) as ordered, no signs or symptoms post injection and patient tolerated well. Vaccine Education provided to parent and what to look out for regarding any reactions. 05/08/2026 Parent presented to the clinic to pick up some paperwork but reported some redness to the vaccine site from previous encounter upon review, there was redness to the administration site, warm to touch. no apparent distress, no fever. Immediately informed provider and added patient to the schedule. Provider examined the patient and prescribed and antibiotic for prophylactic purposes.
