Pt presents for further evaluation left shoulder discomfort after receiving her Hep B series vaccine on 04/09/2026. Pt admits she noticed some discomfort with moving her shoulder, specifically with abduction and extension. No radicular symptoms, paresthesias or weakness. No prior hx of a soft tissue issue.
