Symptom narrative

3rd Dose of HEPLISAV-B Given; Initial report received on 28-Apr-2026. A nurse reported that a 30-year-old female (race and ethnicity unknown) received a third dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization. No medical history or concomitant medications were reported. On 25-Feb-2026, dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot #, expiration date, NDC #, site, and route not reported) was administered. On 26-Mar-2026, dose 2 of HEPLISAV-B (lot #, expiration date, NDC #, site, and route not reported) was administered. On 27-Apr-2026, the patient received dose 3 of HEPLISAV-B (Lot # 946063, expiration date 31-Aug-2027, NDC # 43528-003-05, route not reported) in the left deltoid. The patient was supposed to have a titer drawn but received a third dose of HEPLISAV-B instead. No adverse reactions were experienced. There was no other information available.
