26 Year Old Received An Expired Dose; Initial report received on 30-Apr-2026. A pharmacist reported that a 26-year-old female (race and ethnicity not reported) received an expired dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization. No medical history or concomitant medications were reported. On 04-Mar-2026, the patient received dose 1 of a hepatitis B vaccine of HEPLISAV-B (Lot number 944609, expiration date 29-Jan-2026; NDC number, site, and route not reported). The patient received an expired dose of HEPLISAV-B. No other information was available.
