Symptom narrative

Patient started with quarter sized erythematous rash to injection site on R thigh the night after receiving injection on 5/7/26. Patient rash continued to grow in size over the next two days. Patient was evaluated in clinic on 5/9/26, was told to take Benadryl OTC and use Benadryl lotion as needed and continue to monitor. Patient was evaluated in the ER on 5/10 for adverse reaction. Rash had grown to cover the child's entire thigh and was pruritic per patient's mother. Child was started on Clindamycin, with improvement noted the next day, 5/11/26. Patient evaluated in clinic 5/11/26 for follow up, rash much improved, quarter sized with minor pruritis noted.
