Heplisav-B administered 3 days earlier than 1 month apart; Initial report received on 27-Apr-2026. A pharmacist reported that a patient (age, gender, race, and ethnicity unknown) received the second dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization 3 days earlier than 1 month apart. No medical history or concomitant medications were reported. On an unknown date, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (Lot number, expiration date, NDC #, site, and route not reported). On an unknown date, 3 days earlier than 1 month after having been vaccinated with the first dose of HEPLISAV-B, the patient received the second dose of HEPLISAV-B (Lot number, expiration date, NDC #, site, and route not reported).
