Symptom narrative

Patient was advised to sit on the exam table and lean against the wall for fifteen minutes after the injection. Four minutes after the injections, she felt faint and fell off the table and struck her face on the floor. The fall resulted in a bruise and swelling on her forehead, a superficial laceration that was steri stripped on her forehead and a shallow laceration on her lower lip.
