On an unknown date, the patient was inadvertently vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (lot, expiration date, exact dose, route of administration, and anatomical location were not reported) for prophylaxis instead of the measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live powder and solvent for solution for injection (MMR II) (wrong vaccine administered).
