Symptom narrative

No additional AE; the patient may not have received a full dose; the safety needle came off of the luer-lock as it was being pulled out of the patient's leg / The needle remained in the patient's leg; This spontaneous report was received from a nurse and refers to a patient of unknown age and gender. The patient's concurrent conditions, medical history, previous drug reactions, drug allergies and concomitant therapies were not reported. On 08-May-2026, the patient was vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) 1 Prefilled Syringe 0.5 mL, site of administration reported as leg (unspecified) (route of administration was not provided) lot # Z014461 has been verified to be valid, expiration date: 23-Apr-2027; given as prophylaxis. On the same date, at the end of administration of the vaccine, the safety needle came off of (detached) the luer-lock as it was being pulled out of the patient's leg (Needle issue). The needle remained in the patient's leg and a very small amount of the vaccine liquid squirted into one eye of the patient's father (Captured in case # 2426740). Nothing broke, the product was discarded and was not available for retrieval. The patient might not have received a full dose (Underdose). No symptoms reported, No additional adverse event. This is one of two reports from the same reporter.
