Symptom narrative

Patient attending nursing school. Needs catch up vaccines from childhood. Has no history of prior vaccines. Came in on 3/17/2026 requesting MMR, Varicella, Hepatitis B, and Tdap vaccine. Pt received all 4 without complications. Came back to clinic on 5/7/2026 to receive second doses of MMR, Varicella and Hepatitis B. When reviewing records, noticed pt had received Hepatitis B-Engerix Adult dose 1ml. Lot number YY37B exp. 8/24/2026. Called GSK manufacturer immediately for guidance. Waiting a return call to speak with medication representative with GSK. Patient notified of dosage error.
