Symptom narrative

Student was administered the HPV vaccine into left deltoid. Following student looked uncomfortable. I asked her if she was okay and she said yes. I then gave the Meningococcal vaccine in the right deltoid. Approximately 2 seconds later student laid out backwards onto exam table without response. I noticed slight contraction of her head laterally to the right and her left arm bend up to a 90 degree angle. Then she became relaxed as during shaking over the chest area she woke up. From loss of muscle control to becoming responsive again was less than 5 seconds. Following she felt nausea. She reports feeling dizzy before passing out. She states the second vaccine hurt worse than the first. Denies feeling symptomatic until after the second injection. Student was laid flat with legs elevated. Students symptoms returned to normal within 30 seconds. Blood pressure returned to normal 10 minutes later (see item 19). Student was monitored for a total of 20 minutes and was released once symptoms were gone and vital signs returned to normal.
