Symptom narrative

Patient received the MMR vaccine on Thursday 3/5/26, she developed a rash on the back of her neck on Monday 3/9/26. It generalized over her body over the next several days. She reported it was very itchy. She told the school nurse at her employer and then contacted us. Advised her to see her PCP for rash, she declined. Left multiple messages trying to reach her to confirm all symptoms but received no response.
