Symptom narrative

Nurse confirmed that no symptomatic events occurred; a patient was administered a dose of PROQUAD after it underwent a temperature excursion.; This spontaneous report was received from a Other health professional (nurse) and refers to a patient of unknown age and gender. The patient's medical history, concurrent condition, concomitant therapies were not reported. On an unknown date, the vaccine Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (ProQuad)(lot #Z016962, expiration date: 04-May-2027) 0.5 mL was temperature excursion to 27.6 degrees Fahrenheit for 65 hours 25 minutes 0 seconds. There was no Previous temperature excursion. On 11-May-2026, a dose of this vaccine was administered to the patient for prophylaxis (strength and route of administration were not provided) (Product storage error). Nurse confirmed that no symptomatic events occurred. No additional AE/No PQC reported.
