The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 08-May-2026, the patient started therapy with Hepatitis A Vaccine, Inactivated, Adult (VAQTA) injection (Batch/Lot Number: Z014461) (Expiration date: 23-Apr-2027) (Dose Text: 0.5 mL, total) as prophylaxis. Nurse reported that at the end of administration of the vaccine, the safety needle came off of the luer-lock as it was being pulled out of the his son's leg. The needle remained in his son's leg and a very small amount of the vaccine liquid squirted into one eye of the patient (Device leakage, Device connection issue, Accidental exposure to product) (events about patient's son captured in case# 2426612). Misuse AE being filed for the patient's father. No symptoms reported. No additional AE (No adverse event). This is one of several reports received from the same reporter.
