Syncope

Symptom narrative

Following vaccine administration, patient experienced an episode of syncope. Patient was immediately assessed by the Provider and nurse. Initial vital signs obtained: BP 168/72, HR 60, RR 18, T 98.0, O2 97%, Blood glucose checked and to be 88. Patient was positioned safely, layed down with wet towel and sips of water, and monitored closely. AMR was notified and responded to clinic for further evaluation. Patient was advised to procced to the ER by the Provider, but patient refused to be transported and stated he no longer wanted to go to the ER.
