Symptom narrative

Patient came in requesting Hep B, Pneumonia, Tdap, and Varicella. States she was recently diagnosed with MS and has started her treatment 1 week ago (6 doses of Copaxone 20 mg thus far). States the goal is to start on infusions once she completes vaccines and cancer screenings. Per CDC Pneumonia Vaccine app patient would be eligible for PCV20 (she is a smoker). Dose 1 of Hep B given and scheduled for dose 2. Tdap given. Varicella was also given due to confirming patient is non- immune per blood work. Also, her patient portal showed neurology stating, "completed vaccine series even if she started her injections". Dose 1 of Varicella given; called Neurology (Dr) to get guidance if he wants a second dose completed in 4-8 weeks or not. Spoke with triage nurse who confirmed titer for Varicella was nonimmune and that provider wanted her to complete vaccine series along with cancer screenings. Nurse went farther into her note, after I explained Varicella was a live vaccine and in 4-8 weeks she would be on medication even longer, and found that he had recommended Shingles vaccines not Varicella. Questioned why she would need Shingles vaccine if her Varicella titer was negative. Nurse will speak to provider on next steps and how to handle injections/live vaccine. Patient called and notified of discrepancies and confirmed she has done 6 doses of the Copaxone. VAERS report filled out. RN
