Symptom narrative

On 05/13/26, 16 yo M was seen in clinic for routine child health examination. During this visit, pt was ordered to receive MENACWY-TT (>=2YR) VACCINE (MENQUADFI) and MENB (>=10YR) VACCINE (BEXSERO). Pt had not eaten breakfast prior to arrival, so he was given graham crackers and water before injections. LPN prepared injections. Meningococcal B vaccine 0.5 mL was given IM in left deltoid (Lot #X2TC2, exp: 10/31/2028) and Meningococcal ACWY vaccine 0.5 mL was given IM in left deltoid (Lot #U8660AC, exp: 3/31/2029). ~5 minutes after these injections, pt became pale and leaned over to his side. LPN and pt's father assisted pt to supine position. There was no LOC. MD notified and pt given apple juice and more graham crackers. 10 minutes later, pt vomited x3. Pt transported via wheelchair to UC. At UC, pt's lowest BP was 103/57 with heart rate of 57. Pt acknowledged poor hydration status. Pt administered NS 1000 mL IV bolus and ondansetron 4 mg PO. Pt observerd x 60 minutes with resolution of symptoms. Per PA note: "Strongly favor vasovagal syncope likely compounded by lack of p.o. intake and dehydration". Pt discharged with strict ED/return precautions.
