Symptom narrative

no additional AE; improperly stored dose of ROTATEQ was administered to a patient 4/29/2026; This spontaneous report was received from an office manager and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 29-Apr-2026, the patient was vaccinated with a dose of improperly stored Rotavirus Vaccine, Live, Oral, Pentavalent (Rotateq), Oral solution (lot #2208058, expiration date: 04-Jun-2027) 2 mL (2ml), administered by Oral route for prophylaxis. The vaccine underwent a temperature excursion of 33.7 Fahrenheit for 40 minutes. There was no previous temperature excursion. No additional adverse event was reported.
