Patient was given IPOL and MMR instead of Varivax and MMR. There was miscommunication on which vaccines the patient wanted as his wife was translating for him and she insisted that they fill out the form, even though the other form was also offered, which she had trouble filling out and needed help completing multiple sections. The patient was unable to fill out almost the entire form and could only write and sign his name. Patient did not have an actual adverse event to the vaccine, but was just given IPOL instead of Varivax. Patient's wife came in several days later to state that he was supposed to get Varivax instead of IPOL. The only negative outcome is that the patient's wife is frustrated that he now has to wait for 4 weeks to get the Varivax.
