Symptom narrative

In the fall of 2025, the patient received a COVID vaccine in the right upper arm. She had an immediate onset of pain that has remained since that time. Her symptoms worsened over the next three to four months. She was being seen by a chiropractor who informed the patient she likely had a frozen shoulder. The patient reported her symptoms to her PCP in April and she was referred to Occupational Therapy for treatment. The pain is dull and aching, and can be sharp at times. It is superior and lateral. The presentation according to OT records is suggestive of rotator cuff pathology. She has decreased functional independence and decreased strength and range of motion.
