Symptom narrative

vaccine given in office visit with PCP. Pt left the building and went to school. Dad promptly brought Pt back to the office with symptoms of a headache and shaking. Pt resting in the office with a nurse monitoring. Received oral fluids and a snack. Pt's father gave her Tylenol. PCP saw Pt twice while recovering. V/S stable and Pt feeling better. F/U phone call to Pt and family same afternoon and the next morning. Pt symptoms had resolved.
