Symptom narrative

Patient came to the pharmacy and asked for "any vaccines" she was due for. I checked her chart and saw that she was listed as "due" for Tdap. I printed her form, highlighted this info and gave it to her for her record and administered Boostrix to her. Apparently already had Boostrix administered 08/04/2025. We have attempted to notify her multiple times but she has not returned calls.
