Symptom narrative

5 minutes after vaccine was given the patient stood up to leave. He stopped talking, stepped backwards, and the fell back hitting his head on the wall. On the floor he was unresponsive with decerebrate posturing for about 30 seconds. Once he woke up, he reported a headache. His vitals were slightly abnormal with HR 46, and BP 112/70. 911 was called. Pupils are equal, round and reactive to light. He is alert and oriented x3. Paramedics arrived and rechecked vitals. His vitals stablized and were within normal limits. Due to potenial head injury and no history of seizure activity, we advised he get evaluated at ER. He declined ambulance ride and had his girlfriend come pick him up. She took him to ER for further evaluation.
