On 29-APR-2026, the patient received the 1st dose of Penmenvy. On 29-APR-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate dose of vaccine administered (Verbatim: she did not reconstituted and she gave the syringe portion) and inappropriate preparation of medication (Verbatim: she did not reconstituted and she gave the syringe portion). The outcome of the inappropriate dose of vaccine administered and inappropriate preparation of medication were not applicable. Additional Information: GSK Receipt Date: 05-MAY-2026 Reporter stated that they gave a Penmenvy vaccine but she did not reconstituted and gave the syringe portion, the Men B portion and without the ACWY portion, they wanted to know if the patient could get the ACWY portion with mixing it with a sterile diluent or the administration considered invalid and if she need to be readministered with a full Penmenvy vaccine.
