Skin reaction

Symptom narrative

pt received DTAP and MMR on right arm last visit, she has local skin reaction, Counseled mother that patient has had a localized skin reaction to vaccine, and that this is common in pediatric patients. Recommend mother and patient to continue to monitor. Reaction should resolve with time. May apply cold compress 4 x a day to affected area to aid with resolution of symptoms. May use NSAID as needed to aid with pain and resolution of inflammation. Should lesion spread or develop signs of infection (induration, redness, fluctuance) she should return for medical evaluation. Mother verbalized understanding.
