Symptom narrative

Patient was here for a well visit. At the end of visit, vaccines were reviewed and he was given HPV, TDAP, and Menquadfi. These were his first doses of each vaccine. He remained in the room for a few minutes and was able to walk around. He walked down the halls and then fainted while at the checkout window. Did sustain a witnessed fall with head strike on the floor. He immediately regained consciousness and began crying. Escorted to a room and placed on a table. Endorsed nausea, headache, and neck pain, was diaphoretic. Ice pack placed under back of neck and cool compresses applied to forehead. Assessed by physician; complete neurologic evaluation performed. A circular abrasion was noted to the back, middle of the scalp with a smaller, linear abrasion noted directly below this area. Patient was oriented to person, disoriented to time and place. Asking repetitively the date/time. After approximately 15-20 minutes, patient had continued disorientation. EMS services were initiated for further evaluation in the ER for CT scanning and trauma protocol with concern for concussion. Neck was stabilized with hard C-collar. Patient was transferred in stable condition to ER.
